New insurance patients: You are responsible for making your full co-payment and/or your deductible prior to services being rendered.
New self-pay patients: You are responsible for full payment prior to the services being rendered. No personal checks will be accepted.
Attention All Patients:
We must insist that you bring all medical bottles with you for every appointment. The providers need to see your medications in order to fill them or make change any changes. We are not trying to punish anyone; we just want to be able to provide you with the best medical care possible. Please be advised that if you do arrive without your medications, you will be asked to go get them or reschedule your appointment.
Please turn off cell phones while in exam rooms. Nurses and doctors will not wait for you while you are on the phone. Cell phones can be a health matter for some cardiac patients. You may be asked to reschedule your appointment if you do not comply.
Please have the following items ready at each office visit when called from the reception area: insurance card, photo ID, co-payment, updated personal information and release of medical records (new patients only).
All referral requests need to be made at least 5 business days in advance. The majority have to be requested on-line per your insurance and then can be pending for up to 3 days. Urgent referrals may take 24 – 72 hours. Keep in mind that we do not approve or deny your request; however, we must adhere to the referral guidelines provided by your insurance company. If your primary care physician (PCP) has not diagnosed and treated your illness, you must first make an appointment to be seen before we can refer. To avoid delays, please comply with this request.
Cancellation policy: If you do not call to cancel or reschedule your appointment within 24 hours of the original appointment time, you will be charged a $25 fee that is non-billable to your insurance. This fee must be paid in full by your next appointment.
Co-payment is required at the time of your visit. If you do not remit co-payment at check-in or make arrangements before your appointment time, we will have to reschedule your visit.
